Our nurse practitioner offers assessment, consultation, treatment planning, and follow-up care for adults who want a clearer understanding of their symptoms. 

At Colibri NeuroWellness, our NP services focus on three main areas:

Women’s Health Assessments, Consultations & Follow-Ups

Women’s health support can help you understand changes related to perimenopause, menopause, post-menopause, sexual health, sleep, mood, and daily functioning.

This kind of care can help with hot flashes, night sweats, insomnia, mood changes, anxiety, or irritability, brain fog and memory concerns, and genitourinary syndrome of menopause (including vaginal dryness, burning, itching, or painful intercourse). 

It can also support urinary urgency, frequency, or incontinence, STI testing and sexual health concerns, preventative screening recommendations, and considerations around bone, metabolic, and cardiovascular health.

A nurse practitioner can help assess symptoms, rule out contributing medical factors, and support you with a plan that may include therapy recommendations, medication consultation, lifestyle strategies, diagnostic clarification, or follow-up care.

Adult ADHD can be confusing, especially when you’ve spent years blaming yourself for things that may be connected to executive functioning.

An Adult ADHD assessment can help you better understand how your brain works, whether ADHD is part of the picture, and what support may help.

This can include:

  • A conversation about your current concerns
  • A review of personal, medical, and developmental history
  • ADHD-focused screening questionnaires

Our Approach

How Our NP Services Work

We start with a brief screening call or email with our administrative team to choose the service that fits, whether that’s a Women’s Health, Mental Health, or an Adult ADHD Assessment.

You will be emailed an Intake Form to complete ahead of time so our Nurse Practitioner can use your appointment well. 

If you would like to skip this screening, the Initial Appointment can be scheduled through our Nurse Practitioner’s online booking page, and the Intake Form will be emailed to you to complete.

This is a secure online meeting where you can share your concerns and ask questions in a comfortable, confidential setting.

During this time, your Nurse Practitioner (NP) will listen and learn about your health background and current symptoms, so we can understand your unique needs and experiences. This may also involve discussing your mood, attention, behaviour, and how these concerns affect various aspects of your life, such as work, relationships, and self-care.

 

Your Nurse Practitioner will order any necessary bloodwork or imaging to investigate further into Women’s Health concerns.

For Mental Health or ADHD assessments, this includes the use of standardized rating scales and screening tools to evaluate symptoms, severity, and impact on daily functioning.

During your Follow-Up appointment, you and your Nurse Practitioner will review your results together, discuss any diagnoses, and explore treatment options, including lifestyle changes, medications, and referrals to specialists or other healthcare providers as needed.

Additional Follow-Up Appointments provide an opportunity to reassess your progress, address any new or changing symptoms, and make adjustments to your medication or treatment plan as needed.

Your Nurse Practitioner will work with you to ensure your care remains aligned with your evolving needs and goals, offering guidance and resources to promote long-term health and well-being.

Questions About Our Nurse Practitioner (NP) Services

Adults 18 and older with concerns related to women’s health, mental health, or focus and attention. Common reasons people book include perimenopause symptoms, mood changes, ongoing insomnia, sexual health questions, and curiosity about adult ADHD. If you’re not sure which service is the right fit, our team will help you decide during the screening call.

Yes. Nurse Practitioners in Ontario can diagnose conditions, order tests, prescribe medications, including hormone therapy and ADHD medications where clinically appropriate, and provide ongoing follow-up care.

No. You can book directly. With your consent, we are happy to communicate with your family doctor or other providers so your care stays coordinated.

No. Our NP provides medical assessment, consultation, and ongoing care. Some appointments include a brief supportive conversation, but for psychotherapy, Heather can refer you to one of our psychologists or counsellors, or coordinate with a therapist you already see.

Our Nurse Practitioner-led Adult ADHD Assessment is a focused evaluation designed to help adults get clear answers about ADHD, talk through treatment options, and discuss whether medication might be helpful.

If you need a more detailed evaluation, such as formal documentation for school or work accommodations, or if you think a learning disability might be involved, our Psychologists can provide a comprehensive Psychoeducational or Neuropsychological Assessment.

If you’re not sure which assessment is right for you, our team is here to guide you and help you choose the best option for your needs.

Perimenopause is the transition leading up to menopause, when hormones like estrogen and progesterone start to fluctuate, and your cycle becomes less predictable. Menopause itself is reached once it’s been twelve months since your last period, and the years after are called post-menopause. 

Common signs along the way include cycle changes, hot flashes, night sweats, mood shifts, sleep problems, and vaginal dryness. Because these symptoms can overlap with stress, thyroid issues, or other conditions, our NP will take a full history, order targeted bloodwork if it’s helpful, and help you make sense of what’s hormonal and what might be something else.

The Initial Appointment for NP Assessments take between 60-90 minutes. During this appointment, your NP will ask you questions about your current concerns, your symptom history from childhood to now, and looking at how these symptoms are showing up in your daily life. Your NP will also consider other things that can look like ADHD, including anxiety, sleep difficulties, perimenopause, or chronic stress, so the picture stays accurate. 

Providing Elementary School report cards with teacher comments, or having a family member provide collateral information is welcome but not required.

Follow-Up appointments are scheduled to review your questionnaire responses, lab results, and any tests you may have had. During this appointment, diagnosis (or diagnoses), referrals to other specialists or providers (as needed), and possible options for starting medication will be discussed. Additional Follow-Up Appointments may include medication renewal.
 
You will receive a written summary of your diagnosis (or diagnoses) and a suggested treatment plan within two weeks after your first appointment.
 

Combined ADHD with Women’s Health or Mental Health

  • Virtual Intake Appointment: 90 min.
  • Initial Virtual Follow-Up Appointment: 30 min.
  • Additional Virtual Follow-Up Appointments (as needed): 30-60 min.
 

Women’s Health Only, Mental Health Only, or ADHD Only

  • Virtual Intake Appointment: 60 min.
  • Initial Virtual Follow-Up Appointment: 30 min.
  • Additional Virtual Follow-Up Appointments (as needed): 30-60 min.

After your Initial Appointment, your Nurse Practition may ask you to complete screening questionnaires or tests to investigate Mental Health or ADHD concerns. If you have Women’s Health concerns, she may order medical imaging or bloodwork for you to complete.

During a virtual Follow-Up Appointment, your Nurse Practitioner will go over your results, communicate any diagnosis (or diagnoses) talk through your treatment options, and discuss receiving a brief report or diagnostic letter if appropriate.

Depending on the results, your treatment plan might include lifestyle suggestions, medication, additional testing, ongoing follow-up appointments, or a referral to another provider on our team or elsewhere. Whether you came in with women’s health questions, a mental health assessment, or curiosity about adult ADHD, we’ll help you find clarity around what’s going on and practical next steps you can take. 

There’s no set number. Some people get what they need in a few focused visits. Others stay connected over months as life shifts or treatment is adjusted. Early on, you and your NP will set goals that matter to you, like sleeping through the night, less brain fog at work, or fewer hot flashes, and revisit them at follow-ups so progress is easy to see. Signs things are heading in the right direction often include better sleep, steadier mood, easing insomnia, and feeling more like yourself in daily life. If things stall, your NP can adjust the plan, change medications, or bring in additional support so your care stays flexible.

No. All Nurse Practitioner appointments are offered exclusively through secure virtual visits, available to clients anywhere in Ontario. This makes it easier to access care if you live outside the region, have mobility challenges, or have work hours that don’t fit a typical clinic schedule. If you require medical imaging, bloodwork, or any in-person testing/screening, these will be arranged at a lab or clinic most convenient for you to visit.

If a referral is made to see another healthcare provider at Colibri NeuroWellness or to an external specialist for ongoing care, in-person visits will be coordinated as appropriate.
